New Patient Forms
Please assist us by completing all new patient paperwork before your first visit. These forms are available via download at right by clicking on the NEW PATIENT FORMS BUTTON. If you prefer, we can email the forms to you. If you are unable to print the forms prior to your appointment, you are welcome to arrive a few minutes before your appointment to complete these forms in our cozy waiting room, complete with coffee or tea!
Exam
During your first visit, Dr Sexton will sit down for a complete oral consultation. She will explain any services/procedures recommended as you move forward with your dental health. Dr. Sexton welcomes all questions and concerns at this time. It is her goal to make each patient comfortable while visiting Main Line Smiles.
X-Rays
We will take a complete series of X-rays or check-up x-rays depending on your needs. If you have recently had dental X-rays at another office, please attempt to get a copy sent to Main Line Smiles prior to your appointment. They can be emailed to [email protected]. If you would like us to contact your previous dentist for you, we are happy to do so.
Cleaning
If you are having your teeth cleaned on this day, your appointment will begin with a review of your medical history, followed by a comprehensive cleaning with one of our experienced hygienists.
